Tickets buried on page two. Tickets and licences should sit near the top, with their proper names. A supervisor shouldn't have to hunt for your rigging or HRWL.
Site names with no context. A site name alone doesn't say what you did, what gear you ran or who you worked for.
Missing scope. Equipment, plant, shutdown scopes and the size of the jobs. That's what shows the level you work at.
No roster history. For FIFO roles, showing you've held swings matters.
Safety as one generic line. "Safety conscious" says nothing. Your resume should show how you actually work on site.
